Viability and drug sensitivity of M. leprae isolated from long-term WHO/MDT treated multibacillary leprosy patients.

چکیده
The ALERT Leprosy Control Programme introduced WHOIMDT in Ethiopia in January 1 983 . Paucibacillary patients are treated for 6 months with self-administered daily dapsone and rifampicin once monthly under supervision. Multibacillary patients are treated for a period of at least 2 years or until the skin smears become negative. Treatment consists of self-administered dapsone, clofazimine and rifampicin and clofazimine monthly under supervision. In 1 990, the ALERT leprosy control and the ALERT Hospital Services observed multibacillary patients who were suspected to be unresponsive to the WHOIMDT regimen. The basis of this concern was that by the yearly skin slit examination, the bacteriological index (BI) did not decrease by 1 log unit per year, despite adequate compliance and above 80% attendance rate, with the WHO-recommended MDT. Using the mouse foot pad assay system, we tested for viable bacilli from some of these patients from 1 992 to 1 993 .

Viability of Mycobacterium leprae in skin and peripheral nerves and persistence of nerve destruction in multibacillary patients after 2 years of multidrug therapy.
The pathological changes, bacterial load, and viability of Mycobacterium leprae in the skin and nerves of nine lepromatous leprosy patients who had undergone 2 years of multidrug therapy (MDT) were studied. M. leprae and varying amounts of their remnants were present in the nerves and skin of all but one patient. M. leprae isolated from skin biopsies of six patients and nerve biopsies of nine p...
متن کاملImmunochemotherapy with interferon-gamma and multidrug therapy for multibacillary leprosy.
Treatment for multibacillary leprosy is presently performed with a multidrug therapy (MDT) scheme maintained for 2 years. Leprosy treatment however can benefit from the reduction of length. The lack of interferon-gamma (IFN-gamma) production by lepromatous leprosy (LL) patients' lymphocytes lead us to use this cytokine in the treatment of multibacillary leprosy associated with MDT in the treatm...
متن کاملMolecular determination of Mycobacterium leprae viability by use of real-time PCR.
Mycobacterium leprae, the etiological agent of leprosy, is noncultivable on axenic media. Therefore, the viability of M. leprae for clinical or experimental applications is often unknown. To provide new tools for M. leprae viability determination, two quantitative reverse transcriptase PCR (RT-PCR) assays were developed and characterized. M. leprae sodA mRNA and 16S rRNA were used as RNA target...
متن کاملPresence of M. leprae in tissues in slit skin smear negative multibacillary (MB) patients after WHO-MBR.
This study looked for M. leprae in the lymph node, nerve and skin of multibacillary (MB) leprosy patients who become slit skin smear negative after the completion of WHO-MBR. Twenty-five WHO-MBR-treated multibacillary leprosy patients were studied; borderline lepromatous (BL) leprosy (n = 11) and lepromatous (LL) leprosy (n = 14)). Fifteen patients had reaction (erythema nodosum leprosum 11, up...
متن کاملRelapse with multibacillary leprosy caused by rifampicin sensitive organisms following paucibacillary multidrug therapy.
Many leprosy patients treated with multidrug therapy (MDT) had previously received dapsone (DDS) monotherapy for many years. We report here 2 such patients treated with modified paucibacillary MDT composed of rifampicin and DDS who subsequently relapsed with multibacillary leprosy 5 and 6 years after release from treatment. Isolates of Mycobacterium leprae from both patients were resistant to D...
